Paperwork

We will have paperwork for you to fill out on your first visit. The paperwork provides us with general information about yourself and your condition. Please come 30 minutes prior to your first appointment, or print out the proper forms by clicking on the link below. Please fill these out and bring them with you to your first visit.

Discussion

Once your paperwork has been completed, you will have a consultation with your provider to discuss your health-related problems, concerns and potential treatment options. We will also discuss any goals you might have for your health.
 

Examination

Your provider will ask you various questions related to your condition to determine the problem. They will then use thorough tests and specialized measures to determine which tissues are affected and to what extent. 
 

Treatment

Once the examination is complete, you will be provided with your first treatment session. This may include physical/manual therapies, spinal adjustments, soft tissue therapies, and/or exercise therapies.

Home Program

At the conclusion of the evaluation, you will be given instructions on certain activities or procedures to perform at home. This may include instructions to apply ice or heat to the affected area, avoid certain activities or positions, and/or perform regular home exercises or stretches. 
 

Scheduling

Before you leave, we will review our findings and discuss a treatment plan for your condition. We will then schedule convenient appointment times for your follow-up visits.
